QP and Case Management
Mr. Wycliffe Mokaya serves as CarePoint Home Health’s Qualified Professionals (QPs) and Nurse Case Manager. He has worked as a Registered Nurse since 2005, the same year he received his Associate’s degree in nursing. He went on receive his Bachelor’s degree in Nursing in 2009 and currently has over 12 years of nursing experience in the acute care areas of medical, surgical, cardiac, and orthopedics. Working as a certified nursing assistant in an area nursing home while pursuing his Associates degree helped Wycliffe gain an immense amount of experience caring for geriatric patients as well. He first became interested in nursing because his older sister was a nurse and had always served as a role model to her younger brother. After moving to the United States from Kenya in 1998 and starting college, there was no question what field he would pursue as a profession.
Wycliffe believes the most rewarding aspect of being a registered nurse is knowing that he is making a difference in someone’s life. The patients he cares for often commend him for his skill and thank him for his kindness. In 2013, those accolades went well beyond the bedside when Wycliffe received the “Excellence in Nursing Practice” Award from Fairview Southdale Hospital. This award is given annually to the nurse that demonstrates the highest level of excellence in nursing practice. Future aspirations for Wycliffe focus upon improving the health and well-being of the community around him.
Wycliffe resides in the Southwest Metro of the Twin Cities with is family, and his personal interests include health and wellness, staying fit, and traveling. He loves football and is a huge fan of the Minnesota Vikings.
